Last Updated: September 14, 2026
These Terms of Service (“Terms”) govern your access to and use of the website located at and any webpages, forms, calculators, resources, communications, features, or other online functionality made available through the website (collectively, the “Website”).
These Terms constitute a legally binding agreement between you and (“MedexCore,” “we,” “us,” or “our”).
Please read these Terms carefully before using the Website.
If you do not agree to these Terms, you should not use the Website.
1. About MedexCore
MedexCore provides medical billing, revenue cycle management (“RCM”), medical coding, claim submission, denial management, accounts receivable follow-up, payment posting, insurance eligibility verification, prior authorization support, provider credentialing, reporting, and related administrative services for physicians, medical practices, specialty clinics, group practices, and other healthcare organizations. The Website provides information about MedexCore and its services and allows current and prospective clients to contact us, request consultations, request assessments or audits, and access informational resources.
2. Website Terms vs. Client Agreements
These Terms primarily govern the use of the MedexCore Website. Accessing this Website, submitting an inquiry, requesting a consultation, requesting an audit, receiving a proposal, or communicating with MedexCore does between you and MedexCore.
Actual medical billing, coding, RCM, credentialing, prior authorization, or other professional services will be governed by a separate written agreement, such as a Master Services Agreement, Service Agreement, Statement of Work, Business Associate Agreement, or other written contract entered into between MedexCore and the applicable client. If these Terms conflict with a signed client agreement regarding paid services, the signed client agreement will control with respect to those services.
If these Terms conflict with an applicable Business Associate Agreement regarding Protected Health Information, the Business Associate Agreement and applicable law will control with respect to such Protected Health Information. Nothing contained on this Website modifies any signed agreement between MedexCore and a client unless MedexCore expressly agrees to the modification in a written instrument signed by an authorized representative.
3. Eligibility and Authority
You may use this Website only if you are legally capable of entering into a binding agreement. If you access or use the Website on behalf of a medical practice, healthcare organization, company, employer, or other legal entity, you represent and warrant that you have authority to bind that organization to these Terms where applicable. You may not use the Website if your use would violate applicable law or if you have previously been prohibited from accessing the Website by MedexCore.
4. Informational Purpose
Information appearing on the Website is provided for general business and informational purposes. Website content does not constitute:
- medical advice;
- diagnosis or treatment advice;
- legal advice;
- tax advice;
- accounting advice;
- insurance advice;
- clinical advice;
- coding advice for a specific patient unless provided pursuant to a contracted service;
- a guarantee of insurance coverage;
- a guarantee of payer reimbursement;
- a guarantee of claim approval;
- a guarantee of collections;
- a guarantee of regulatory compliance; or
- a substitute for professional advice based on the facts of a particular situation.
Healthcare regulations, coding rules, payer policies, reimbursement requirements, and government program requirements can change frequently. You should not make clinical, financial, compliance, legal, or reimbursement decisions solely on the basis of general Website content.
5. No Provider-Patient Relationship
MedexCore provides administrative and revenue cycle services to healthcare organizations. MedexCore does not provide medical diagnosis or treatment through this Website and does not establish a physician-patient or other healthcare provider-patient relationship with Website visitors. Patients should contact their physician, healthcare provider, health plan, or other appropriate healthcare organization concerning medical treatment, diagnosis, coverage, medical records, billing disputes, or patient-specific healthcare questions. The Website is not intended for medical emergencies. If you are experiencing a medical emergency, call 911 or seek appropriate emergency medical assistance.
6. No Guarantee of Financial or Billing Results
Medical billing and revenue cycle outcomes depend on many circumstances outside MedexCore's control. These may include:
- payer policies;
- insurance coverage;
- patient eligibility;
- benefits;
- contractual reimbursement rates;
- medical necessity requirements;
- provider documentation;
- coding requirements;
- timely filing deadlines;
- payer processing times;
- claim-editing systems;
- payer audits;
- government program requirements;
- prior authorization rules;
- credentialing status;
- coordination of benefits;
- patient responsibility;
- deductibles and copayments;
- appeals decisions;
- clinical information supplied by the provider;
- changes in applicable law; and
- actions or omissions of third parties.
Accordingly, statements concerning potential revenue improvement, collection improvement, denial reduction, accounts receivable performance, clean claim rates, reimbursement, savings, turnaround time, or similar performance measures are goals, estimates, benchmarks, illustrations, or historical observations unless a separate signed agreement expressly states otherwise.
Actual results vary by client, specialty, payer mix, documentation quality, workflow, patient population, location, contract terms, and other circumstances.
7. Medical Billing and Claims
Where MedexCore is engaged under a separate written agreement to perform medical billing services, MedexCore may prepare, process, transmit, monitor, correct, follow up on, or otherwise assist with claims based upon information and documentation supplied or made available by the client. Healthcare providers remain responsible for ensuring that services actually provided to patients are truthfully and adequately documented. Clients must not knowingly provide false, incomplete, misleading, altered, fraudulent, or unsupported information for use in a claim. MedexCore will not knowingly submit a claim that MedexCore reasonably believes is fraudulent, materially false, unsupported by available documentation, or prohibited by applicable law. We may request clarification or additional documentation where information appears incomplete, inconsistent, or insufficient for billing or coding purposes. Nothing in these Terms requires MedexCore to submit or continue processing a claim that MedexCore reasonably believes would violate applicable law, payer requirements, government program requirements, professional coding standards, a client agreement, or MedexCore's compliance obligations.
8. Provider Documentation and Medical Necessity
The healthcare provider is responsible for creating and maintaining appropriate clinical documentation regarding the services furnished to patients. Unless otherwise expressly agreed in writing, MedexCore does not independently determine:
- whether healthcare services were actually rendered;
- whether a service was medically necessary;
- whether clinical documentation accurately reflects patient care;
- whether the provider was appropriately licensed or qualified to furnish the service;
- whether clinical supervision requirements were satisfied; or
- whether information supplied by the provider is clinically accurate.
MedexCore may identify apparent documentation deficiencies, request clarification, recommend corrective action, or assist clients with administrative compliance processes.
9. Medical Coding
Where MedexCore provides coding services under a separate agreement, coding will be based upon available medical records, provider documentation, applicable coding guidelines, payer requirements, and the scope of services agreed upon by the parties. We will not knowingly engage in practices such as unsupported upcoding, unbundling, assumption coding, falsification of documentation, billing for services not rendered, or other improper coding practices. Healthcare providers must provide sufficient, timely, accurate, and authentic documentation necessary to support appropriate coding. Where clinical documentation is ambiguous or insufficient, MedexCore may request clarification rather than assume unsupported clinical facts. Coding recommendations or determinations do not guarantee payment by any payer. The final payment or coverage decision remains with the applicable payer or governmental program.
10. Payer Decisions
MedexCore does not control insurance companies, Medicare, Medicaid agencies, managed-care organizations, workers' compensation programs, clearinghouses, or other payers. A payer may deny, reduce, suspend, recoup, audit, delay, or otherwise adjust payment for reasons outside MedexCore's control. MedexCore cannot guarantee that a payer will:
- accept a claim;
- pay a claim;
- pay a particular amount;
- approve an appeal;
- recognize a particular code;
- approve medical necessity;
- process a claim within a specific period;
- accept documentation;
- enroll or credential a provider;
- approve a prior authorization; or
- maintain its policies or reimbursement rates.
Any payer decision remains subject to that payer's policies, contracts, regulatory requirements, appeals procedures, and applicable law.
11. Prior Authorization
Prior authorization services are administrative support services. Submission of a prior authorization request does not guarantee:
- approval;
- coverage;
- medical necessity determination;
- reimbursement;
- eligibility;
- payment;
- continued coverage; or
- a particular reimbursement amount.
Even when prior authorization is granted, payment can depend upon additional requirements, including eligibility on the date of service, accurate coding, appropriate documentation, medical necessity, contractual provisions, benefit limitations, and payer rules.
12. Eligibility Verification
Eligibility or benefit information obtained from insurance companies, payer portals, clearinghouses, automated systems, or other sources reflects information available at the time of verification. Eligibility verification is not a guarantee of payment. Coverage may change, terminate, contain exclusions, require authorization, depend upon coordination of benefits, or otherwise differ from information initially provided by a payer. MedexCore is not responsible for inaccuracies in information supplied by insurance companies, patients, clearinghouses, employers, government programs, or other third-party sources except to the extent liability cannot legally be excluded.
13. Provider Credentialing
Credentialing, enrollment, revalidation, contracting, and payer enrollment decisions are controlled by healthcare plans, networks, government agencies, credentialing bodies, and other third parties. MedexCore may assist with preparation, submission, tracking, follow-up, and related administrative processes. MedexCore cannot guarantee:
- acceptance into a payer network;
- provider enrollment;
- credentialing approval;
- a particular effective date;
- retroactive participation;
- reimbursement during the credentialing period;
- processing within a particular timeframe; or
- continuation of payer participation.
Clients are responsible for promptly providing accurate and complete information, signatures, licenses, certifications, attestations, and other documentation required by credentialing organizations.
14. Accounts Receivable and Collections
Where MedexCore provides accounts receivable follow-up, patient billing support, or related services, the recoverability of any balance depends upon circumstances outside MedexCore's control. MedexCore does not guarantee collection of any particular balance or percentage of accounts receivable. Clients remain responsible for establishing lawful financial policies and determining when balances should be adjusted, written off, refunded, transferred, or otherwise handled, except to the extent a separate written agreement assigns particular functions to MedexCore. Any patient communication, collection activity, refund processing, or credit-balance activity will be subject to applicable law and the applicable client agreement.
15. Billing Audits and Assessments
MedexCore may offer complimentary or paid billing audits, revenue cycle assessments, claim analyses, estimates, reports, consultations, or similar evaluations. Unless otherwise agreed in writing, such evaluations are based upon information made available to MedexCore and are not comprehensive legal, regulatory, financial, or compliance audits. An audit or assessment may identify potential issues but cannot guarantee that every billing error, coding issue, compliance concern, underpayment, overpayment, denial, missed charge, security issue, or revenue opportunity will be identified. Clients should not interpret an audit result as certification that their billing operations comply with every federal, state, payer, contractual, or professional requirement.
16. Website Calculators and Estimates
Any calculator, estimator, benchmark, projection, savings figure, reimbursement estimate, revenue estimate, denial calculation, or financial illustration made available through the Website is provided for general informational purposes only. Outputs depend on the information entered and assumptions used. Actual results may vary materially. Calculator outputs do not constitute guarantees, offers, contractual commitments, financial advice, reimbursement determinations, or representations that a particular result will be achieved.
17. HIPAA and Protected Health Information
When MedexCore performs services for a HIPAA Covered Entity and creates, receives, maintains, or transmits Protected Health Information (“PHI”) on behalf of that Covered Entity, MedexCore may act as a Business Associate under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), as amended, including applicable provisions of the Health Information Technology for Economic and Clinical Health Act (“HITECH”). In such circumstances, the parties will enter into an appropriate Business Associate Agreement where required by law. PHI will be used and disclosed as permitted by the applicable Business Associate Agreement, client agreement, HIPAA, and other applicable law. These Website Terms are not intended to serve as a substitute for a Business Associate Agreement. If an applicable Business Associate Agreement conflicts with these Terms regarding PHI, the Business Associate Agreement will control.
18. Do Not Submit PHI Through General Forms
The MedexCore Website's ordinary contact forms, consultation forms, newsletter forms, marketing forms, and standard email channels may not be intended for transmission of patient PHI. Unless MedexCore specifically instructs you otherwise through an approved secure communication method, When MedexCore provides a secure portal, encrypted communication method, approved file-transfer system, or other specifically designated channel for PHI, users should use that method. You are responsible for using the communication method designated by MedexCore for sensitive information.
19. Client Authorization to Provide Information
If you provide MedexCore with patient information, claims information, healthcare records, business information, or other information on behalf of another person or organization, you represent that you have lawful authority to provide that information to MedexCore for the applicable purpose. Healthcare organizations are responsible for obtaining any authorization, consent, permission, or other legal basis required for information they direct MedexCore to process, except where applicable law or a signed agreement allocates that responsibility differently.
20. Healthcare Fraud and Compliance
MedexCore does not knowingly participate in fraudulent, abusive, deceptive, or unlawful billing activity. Users and clients must not use MedexCore's Website or services to facilitate:
- claims for services not actually provided;

- improper duplicate billing;
- fraudulent credentialing;
- unlawful kickbacks;
- improper referrals;
- circumvention of payer rules;
- concealment of known overpayments; or
- other conduct prohibited by applicable healthcare laws or regulations.
MedexCore reserves the right, consistent with applicable contracts and law, to decline instructions, suspend affected activities, request additional documentation, escalate compliance concerns, or take other lawful action where MedexCore reasonably believes requested conduct may violate applicable law, payer requirements, or contractual obligations. Nothing in these Terms requires MedexCore to participate in conduct that MedexCore reasonably believes to be unlawful.
21. Compliance With Laws
Users are responsible for complying with laws and regulations applicable to their own conduct and organizations. Depending upon the circumstances, such laws may include HIPAA, HITECH, the False Claims Act, federal healthcare program requirements, state insurance and healthcare laws, privacy laws, consumer protection laws, coding and billing rules, payer requirements, and other applicable standards. Some jurisdictions do not permit certain warranty exclusions. In those jurisdictions, exclusions apply only to the maximum extent legally permitted. This section relates primarily to the Website. Warranties, if any, applicable to contracted professional services will be governed by the applicable signed client agreement.
36. Limitation of Liability
This includes, where legally permitted, damages for lost profits, lost revenue, lost business opportunities, loss of data, loss of goodwill, business interruption, or similar losses, even if MedexCore has been advised of the possibility of such damages.
This Website limitation does not replace a limitation-of-liability provision contained in a signed client agreement. Liability relating to contracted medical billing, coding, RCM, credentialing, or other professional services will be governed by the applicable signed agreement. Nothing in these Terms excludes or limits liability that cannot lawfully be excluded or limited.
37. Indemnification
To the maximum extent permitted by applicable law, you agree to defend, indemnify, and hold harmless MedexCore and its affiliates, officers, directors, employees, agents, contractors, successors, and permitted assigns from third-party claims, liabilities, losses, damages, judgments, penalties, costs, and reasonable attorneys' fees arising from or relating to:

- your infringement of another person's intellectual property or privacy rights;

This provision does not require you to indemnify MedexCore for liability caused solely by MedexCore's own fraud, willful misconduct, or other conduct for which indemnification cannot lawfully be required.
38. Suspension and Termination
MedexCore may suspend, restrict, or terminate Website access where reasonably necessary because of:
- violation of these Terms;
- security concerns;
- suspected fraud;
- unauthorized access;
- misuse of the Website;
- legal requirements;
- risk to MedexCore or other users; or
- conduct reasonably believed to threaten Website integrity.
Termination of Website access does not automatically terminate any separate client agreement unless that agreement provides otherwise.

This provision does not excuse any legal obligation that applicable law does not permit the parties to waive.
40. Governing Law
These Terms and disputes arising from the Website will be governed by the laws of the State of Texas, without regard to conflict-of-laws principles, except to the extent federal law applies. The Federal Arbitration Act will govern the interpretation and enforcement of any arbitration provision contained in these Terms. A separate signed client agreement may establish a different governing-law provision for disputes relating to contracted services.
41. Informal Dispute Resolution
Before initiating arbitration or litigation arising solely from these Website Terms, you and MedexCore agree to make a reasonable good-faith effort to resolve the dispute informally. A party seeking to initiate a dispute should provide written notice describing:
- the party's name and contact information;
- the nature of the dispute;
- relevant facts;
- the relief requested; and
- sufficient information to allow the other party to investigate.
Notices to MedexCore may be sent to with the subject line The parties will have thirty (30) days after receipt of a sufficiently detailed notice to attempt informal resolution before filing an arbitration demand, unless immediate relief is legally necessary.
42. Binding Arbitration
Except for disputes excluded below or disputes governed by a separate signed agreement, any dispute, claim, or controversy arising out of or relating to these Terms or your use of the Website that cannot be resolved through the informal process above will, to the extent permitted by law, be resolved through binding arbitration rather than in court. The arbitration will be administered by the under the AAA rules applicable to the nature of the dispute at the time arbitration is commenced. The arbitration will be conducted by a single neutral arbitrator. Unless applicable law or AAA rules require otherwise, the arbitration will take place in Austin, Texas, or may be conducted remotely by agreement or where permitted by applicable rules. The arbitrator may award any individual remedy that would otherwise be available in a court of competent jurisdiction, subject to these Terms and applicable law. Judgment on the arbitration award may be entered in any court having jurisdiction. Nothing in this section prevents either party from:
- bringing an eligible individual action in small claims court;
- seeking temporary or preliminary injunctive relief where reasonably necessary to protect intellectual property, confidential information, data security, or prevent unauthorized system access; or
- pursuing relief before a governmental or regulatory authority where applicable law permits.
If you have entered into a separate written client agreement with MedexCore containing a dispute-resolution provision, that provision will control disputes arising from the contracted services covered by that agreement.
43. Class Action and Jury Trial Waiver
Unless both parties agree otherwise, an arbitrator may not consolidate the claims of multiple parties or preside over a representative or class proceeding.
If the class-action waiver is found unenforceable as to a particular claim and that determination cannot be severed from the claim, that claim will proceed in a court of competent jurisdiction rather than class arbitration.
44. Arbitration Opt-Out
You may opt out of the arbitration provision in these Terms by sending written notice to: within thirty (30) days after the date you first affirmatively accept these Terms through a Website mechanism that requires such acceptance. The notice must include your full name, business name if applicable, email address, and a clear statement that you wish to opt out of the arbitration provision in the MedexCore Website Terms of Service. Opting out of arbitration will not affect the remaining provisions of these Terms. Any arbitration provision contained in a separate signed client agreement remains governed by that agreement.
45. Court Venue Where Arbitration Does Not Apply
Where a dispute arising solely from these Website Terms is not subject to arbitration, and where applicable law permits contractual venue selection, the parties consent to the exclusive jurisdiction of the state and federal courts located in or serving . Each party waives objections based on personal jurisdiction or inconvenient forum to the maximum extent permitted by law. A separate client agreement may provide for a different jurisdiction or venue.
46. Time Limit for Website Claims
To the extent permitted by applicable law, a claim arising solely from use of the public Website must be commenced within one (1) year after the cause of action accrued or within the minimum period that applicable law permits the parties to contractually establish, whichever is longer. This limitation does not apply where applicable law prohibits contractual shortening of the applicable limitations period. Claims arising under a signed client agreement remain subject to that agreement and applicable law.
47. Changes to the Website
MedexCore may update, modify, replace, add, or remove Website content or functionality at any time. Website descriptions of services may change as our offerings, technologies, regulations, payer requirements, or business operations evolve. The availability of a service described on the Website does not guarantee that MedexCore will offer that service to every organization.
48. Changes to These Terms
MedexCore may revise these Terms from time to time. When we do so, we will update the “Last Updated” date shown at the beginning of the Terms. Material changes will apply prospectively to the extent required by applicable law. Where applicable law requires additional notice or affirmative acceptance of a material change, MedexCore will provide such notice or obtain such acceptance. Continued use of the Website after revised Terms become effective constitutes acceptance where legally permitted. Changes to these Website Terms do not automatically amend an existing signed client agreement.
49. No Waiver
A failure by MedexCore to enforce any provision of these Terms on one occasion does not waive our right to enforce that provision or any other provision later. A waiver is effective only if made in writing by an authorized MedexCore representative.
50. Severability
If a provision of these Terms is held invalid, unlawful, or unenforceable, that provision will be enforced to the maximum extent legally permitted and, where possible, modified or severed without affecting the validity of the remaining provisions. Any special severability rules stated in the arbitration or class-action sections will control with respect to those provisions.
51. Assignment
You may not assign or transfer your rights or obligations under these Website Terms without MedexCore's prior written consent. MedexCore may assign these Terms in connection with a merger, acquisition, corporate reorganization, sale of substantially all relevant assets, change of control, or by operation of law, subject to applicable legal obligations. Assignment of a separate client agreement will be governed by that agreement.
52. No Partnership or Agency
Your use of the Website does not create a partnership, joint venture, employment relationship, franchise, fiduciary relationship, or agency relationship between you and MedexCore. No Website visitor has authority to bind MedexCore. Any agency or authorization necessary for MedexCore to perform contracted billing or administrative services will be governed by a separate written agreement and applicable law.
53. No Third-Party Beneficiaries
Except where expressly stated in these Terms or required by law, these Terms do not create rights in any person or entity that is not a party to them.
54. Entire Agreement Regarding Website Use
These Terms, together with the Privacy Policy and any additional Website-specific terms expressly incorporated by reference, constitute the agreement between you and MedexCore regarding use of the public Website. They supersede prior understandings concerning Website use only. They do not supersede a Master Services Agreement, Business Associate Agreement, Statement of Work, confidentiality agreement, data processing agreement, or other separate written contract between MedexCore and a client.
55. Headings
Section headings are provided for convenience only and do not limit or otherwise affect the interpretation of these Terms.
56. Interpretation
Words such as “including” and “include” are intended to mean “including without limitation” unless the context clearly requires otherwise. References to laws and regulations include applicable amendments, replacements, implementing regulations, and successor provisions where appropriate. Nothing in these Terms will be interpreted to waive a right or obligation that cannot lawfully be waived.
